Job summary

UnitedHealth Group is seeking an RN Case Manager for hospice care coordination, working with an interdisciplinary team to assess patients and develop individualized plans. You will support patients and families, ensuring compassionate care and ongoing communication.

The role emphasizes on-call duties, thorough documentation, and collaboration with the care team to enhance patient outcomes in a home-based hospice setting.

Qualifications

• Current RN license, state(s) you are assigned to work
• Current CPR certification
• 1+ years of nursing experience
• Valid driver's license, reliable transportation and liability insurance

Responsibilities

• Performs patient assessments with the care team to develop and implement a plan of care
• Makes referrals to hospice care disciplines as needed or documents rationale if not
• Maintains patient care progress records with accuracy and timeliness
• Records pain and symptom management changes/outcomes
• Teaches patients, families, and caregivers appropriate care techniques
• Regularly communicates with the clinical manager and care team
• Performs on-call responsibilities for patients/families as assigned
• Other duties as needed
