The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to coordinates patient care, manage plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life.

Qualifications

• Active RN License is required.
• Hospice nursing experience is preferred.
• Experience with Homecare Homebase (HCHB) is preferred.
• Strong assessment, communication, and care coordination skills are essential.

Responsabilités

• Provide skilled nursing care, assessments, and symptom management per hospice standards and physician orders.
• Complete scheduled visits, admissions, and clinical interventions with documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care with compassionate support.
