Infinity Hospice is seeking a dedicated RN Case Manager to join our home-based care team in Pahrump, Nevada. You will lead patient care in the home setting, assess needs, develop plans, and coordinate with the Interdisciplinary Group to ensure comfort and dignity at every stage.

You’ll collaborate with physicians and families, document plans, follow infection control best practices, and contribute to a culture of compassionate, patient-centered care within a team that truly works together.

Qualifications

• Two years RN experience in a clinical setting and one year full-time hospice.
• Graduate of Diploma, Associate, or Baccalaureate School of Nursing.
• Current State RN license; valid driver’s license.

Responsabilités

• Admit patients and provide nursing care in patients' homes.
• Coordinate care with the IDG, patient, family, and referring agencies.
• Assess patients using Nursing Assessment and LCD criteria.
• Develop and document individualized plans of care.
• Provide skilled nursing care with infection control practices.
• Coordinate with physicians involved in patient care.
• Communicate clearly with patients, families, team, and other professionals.
• Contribute to quality via the hospice QAPI program.
• Share the hospice philosophy of comfort, dignity, and patient-centered care.
