Infinity Hospice Care LLC is seeking an RN Case Manager to provide comprehensive hospice nursing in patients' homes in Reno, NV. You’ll admit patients, coordinate care, and partner with the IDG to meet the needs of patients and families during a pivotal time.

You will assess physical, psychosocial, and environmental needs, develop individualized plans, and deliver skilled nursing with proper infection control, working closely with physicians and the care team to ensure comfort and dignity.

Formación

• Active RN license in the United States.
• Experience delivering end-of-life or hospice care.
• Ability to work in patients' homes and coordinate care.

Responsabilidades

• Assess patients thoroughly and identify physical, psychosocial, and environmental needs.
• Develop and document individualized plans of care based on assessments.
• Provide skilled nursing care and infection control practices.
• Partner with the Interdisciplinary Group (IDG) to meet patient and family needs.
• Coordinate with physicians involved in each patient’s care.
• Communicate clearly and compassionately with patients, families, and healthcare staff.
• Contribute to quality through the hospice’s QAPI program.
• Share the hospice philosophy of comfort, dignity, and patient-centered care.
