Job summary

The Pennant Group, Inc. operates Comfort Hospice Care and is seeking a PRN Hospice RN to provide in-home care in the Pahrump area.

You will act as a trusted case manager, delivering compassionate end-of-life care and guiding families through care decisions. You will complete assessments, develop plans, administer medications, educate families, and document care while joining an interdisciplinary team and participating in on-call rotations.

Qualifications

• Current RN license in the State of Arizona
• Minimum of 2 years of nursing experience, hospice or home health experience preferred
• Homecare Homebase (HCHB) experience preferred, not required
• Valid drivers' license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
• Ability to work independently and a passion for making a real difference

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs
• Develop and implement individualized care plans, revising as needed
• Provide skilled nursing care, administer medications, and manage symptoms
• Educate patients and families on care techniques, medications, and comfort measures
• Maintain accurate clinical documentation and communicate changes to physicians
• Participate in interdisciplinary team meetings and quality improvement initiatives
• Provide emotional support and counseling to patients and families
• Supervise hospice aides and delegate responsibilities appropriately
• Collaborate with an interdisciplinary team that shares your commitment to excellence
• Participate in on-call rotation as needed
