Care Initiatives Hospice is seeking a compassionate Registered Nurse (RN) to join as an RN Case Manager. You will meet patients and families where they are—at home, long-term care, assisted living, or hospital—delivering comprehensive, patient-centered care during life-limiting illness.

You will develop and oversee plans of care with the physician, perform assessments, provide counseling to families, document thoroughly, and coordinate with an interdisciplinary team to meet each patient’s unique

Qualifications

• Active NE RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Two (2) years of verifiable nursing experience (RN/LPN preferred).
• Valid driver’s license and auto liability insurance; ability to travel within region.
• Home internet access.
• Ability to obtain CHPN within 24 months.

Responsabilités

• Use nursing knowledge to deliver high quality patient care at the patient’s residence.
• Develop and oversee plan of care under physician direction with interdisciplinary involvement.
• Perform initial and comprehensive patient assessments according to policies.
• Continual visits and interdisciplinary assessment of the patient.
• Deliver clear, concise clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated.
• Develop and continually update the plan of care to meet patient-specific needs.

CHPN eligibility within 24 months
