Job summary

Gentiva seeks a Hospice RN / RN Case Manager to provide in-home hospice and palliative care. You will perform comprehensive nurse assessments, deliver symptom management, and develop personalized plans of care in patients' homes or other settings.

You will educate families, coordinate with the IDG, document care, and ensure regulatory compliance while supporting patients and loved ones through end-of-life care.

Qualifications

• RN with active state license.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments in person or by telehealth.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams including physicians and nurses.
• Educate patients and families on disease, end-of-life care, medications, and self-care.
