Gentiva is seeking a Hospice RN / RN Case Manager to provide skilled nursing care in patients' homes, assisted living, or other settings. You will conduct initial and ongoing assessments, implement and adjust plans of care, and collaborate with an interdisciplinary team to ensure compassionate, high-quality hospice services.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year of hospice or home health experience.
• Current CPR certification.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments in home or patient setting.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care with IDG and physicians to refine care plans.
• Educate patients and caregivers on disease, care and end-of-life needs.
