Job summary

JourneyCare Hospice is seeking a Hospice RN to join our team in Arlington Heights, IL. The role handles patient and family care from intake through evaluation, with regular visits and plan updates.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR and First Aid certifications required.
• Knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Own and manage patient and family caseload across assessment, admission, planning, implementation and evaluation.
• Gather data from patient factors (physical, psychological, social, spiritual) to inform care.
• Coordinate care by communicating with attending physician and interdisciplinary team.
• Provide regular visits to reassess needs and update the plan of care.
• Provide pronouncement of death and family support.
• Maintain accurate patient records, charts, progress notes, and care plans.
• Supervise and document delivery of home care by Home Health Aide as required.
