Addus HomeCare is seeking a Hospice Registered Nurse Case Manager to join JourneyCare Hospice in Marengo, IL. The role involves patient and family care management, interdisciplinary coordination, and ensuring high-quality home health services.

Salary ranges from $80,000 to $97,000 annually based on experience, with strong benefits, time off, and professional development opportunities. Monday–Friday with occasional weekend coverage and comprehensive company support.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Lead patient and family nursing care from assessment through plan and evaluation.
• Coordinate with interdisciplinary team and external agencies to ensure optimal care.
• Visit patients regularly to reassess needs and update the care plan.
• Document records timely and supervise Home Health Aide delivery as required.
