Job summary

JourneyCare Hospice in Marengo, IL is seeking a Hospice Registered Nurse to join our team. The role offers competitive salaries, generous time off, and a comprehensive benefits package within a supportive culture.

Responsibilities include patient/family assessment, care planning, coordination with the interdisciplinary team, and regular home visits. A valid IL RN license, CPR/First Aid, and driving privileges are required.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certifications current.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility determination, admission processing, planning, implementation and evaluation phases of the nursing process.
• Obtain data from patient and family across physical, psychological, social and spiritual factors affecting needs and initiate intervention by the team.
• Initiate communication with the attending physician, interdisciplinary team and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate patient records, charts, progress notes, and timely care plans.
• Supervise and document delivery of care by Home Health Aide per guidelines.
