Job summary

JourneyCare Hospice in Arlington Heights, IL seeks a compassionate Hospice RN to join our team. You will assess patients, determine eligibility, process admissions, and plan care across the nursing process in the home setting.

Collaborating with the interdisciplinary team and physicians, you will visit patients regularly, document progress, and support families through care transitions. We offer a competitive salary, generous time off, 401(k) with match, mileage reimbursement, and tuition

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certifications current.
• Knowledge of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance.

Responsibilities

• Assume primary responsibility for patient and family caseload through assessment, eligibility, admission processing, planning, implementation and evaluation.
• Obtain data from patient factors (physical, psychological, social, spiritual) impacting needs and coordinate with the team.
• Initiate communication with physicians, interdisciplinary team, and other agencies for optimal care.
• Provide regular visits to reassess needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts, progress notes, and timely care plans.
• Supervise and document delivery of care by Home Health Aide per guidelines.
