Job summary

Floyd Healthcare Management, Inc. seeks a Hospice Nurse Case Manager (RN) to provide skilled nursing care and coordinate interdisciplinary services in patients' homes across Floyd, Chattooga, Gordon, and surrounding counties.

This full-time role includes on-call rotations and travel within the service area. The ideal candidate holds an accredited RN credential, Georgia licensure, and strong assessment, communication, and care planning skills, with prior hospice or home health experience

Qualifications

• Graduate of an accredited RN program required.
• Georgia RN license and valid driver_license required.
• Experience in home health or hospice care preferred.

Responsibilities

• Conduct admissions, pre-admission, routine, urgent, and pronouncement visits.
• Develop and revise individualized patient care plans.
• Provide direct nursing care and symptom management at home.
• Coordinate with interdisciplinary team to support patient and family.
• Maintain accurate clinical documentation.
