Job summary

ExcelaCare Home Health is seeking a dedicated Registered Nurse to deliver hospice care at patients’ homes and facilities. You will coordinate with a multidisciplinary team, develop and implement the Plan of Care, and educate families on care options and symptom management.

The role requires current RN licensure, CPR certification, and the ability to travel locally; on-call and visits are part of the schedule.

Qualifications

• Education: Graduate of an accredited school of nursing.
• Experience: Two years RN experience in a clinical setting; hospice preferred.
• License/Certification: Current valid RN license; CPR certification; valid driver's license; no OIG exclusions.

Responsibilities

• Administers medications and treatments per agency policy and standards.
• Develops and implements a written Plan of Care with patient and family, per physician orders.
• Documents patient status timely and accurately; updates care plan as needs change.
• Coordinate care with multidisciplinary team and referring agencies.
• Provide patient/family education on health plans, symptom control, and end-of-life care.
• Participate in on-call rotation and complete scheduled visits.
