Job summary

Virtua is seeking a Hospice Registered Nurse Case Manager to provide compassionate end-of-life care to patients in the New Jersey hospice service area. You will assess needs, educate families, and coordinate with physicians and an interdisciplinary team to develop individualized care plans.

Responsibilities include medication administration, symptom management, and documenting care plans in the patient’s chart.

Qualifications

• Minimum 2-3 years of hospice nursing experience.
• Graduate of an accredited nursing program.
• Current NJ nursing license and CPR/BLS.
• Ability to work independently in home settings.

Responsibilities

• Assess patient comfort and needs in home setting.
• Educate families on symptom management and prognosis.
• Develop individualized care plans with an interdisciplinary team.
• Administer medications and monitor for changes.
• Participate in bi-weekly interdisciplinary team meetings.
• On-call, weekend, and holiday shifts may be required.
• Travel statewide to patient homes.
