Job summary

Virtua Health is seeking a Hospice RN Case Manager to deliver compassionate end-of-life care in patients’ homes across New Jersey. The role emphasizes assessment, symptom management, and family education within an interdisciplinary team.

The RN will coordinate care plans, order medications and equipment, and participate in bi-weekly team meetings. On‑call, weekend, and holiday shifts may be required, with a 40-hour work week and on-site work in Lippincott Drive region.

Qualifications

• Minimum of 2-3 years of hospice nursing experience.
• Graduate of an accredited nursing program.
• Current State of New Jersey nursing license to practice as a registered nurse. CPR/BLS certification.

Responsibilities

• Provide compassionate hospice care to patients in their home setting.
• Assess and manage symptoms, educate families on prognosis and symptom management.
• Collaborate with physicians and interdisciplinary team; coordinate care and referrals.
• Participate in on-call, weekend and holiday coverage as required.
• Maintain accurate documentation per agency policies.
