Job summary

Hospice of the Valley in Arizona seeks an RN Case Manager to provide and coordinate hospice care for home care patients. You will lead the interdisciplinary team to ensure patients' physical, emotional and spiritual needs are met, educate families, and manage symptoms in the patient’s home or in a facility where care is provided.

This role emphasizes compassionate, patient-centered care with strong assessment skills and collaboration across disciplines.

Qualifications

• Graduate of accredited nursing program.
• Current Arizona RN licensure in good standing.
• Minimum one year of medical, surgical or critical care nursing.
• Strong clinical assessment skills.
• Excellent communication and interpersonal skills.
• Solution-driven, creative and resourceful problem-solving skills.
• Ability to manage time well and perform duties with accuracy.
