Job summary

Hospice of the Valley in Arizona's East Valley is seeking an experienced RN Case Manager to join the AZSC team. The role focuses on care coordination, in-home visits when appropriate, and collaboration with physicians to improve quality of life for seriously ill patients.

You will assess patients, plan care, teach families, document accurately, and adhere to professional standards. Minimum qualifications include RN license, OASIS C experience, and strong clinical communication.

Qualifications

• Graduate of accredited school of nursing.
• OASIS C experience.
• Minimum one year of recent experience in medical, surgical or critical care as a professional nurse.
• Strong clinical assessment skills.
• Excellent communication and interpersonal skills.
• Solution driven, creative and resourceful problem solving skills.
• Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through.
