Job summary

Hospice of the Valley is seeking an RN Case Manager for the Arizona Supportive Care (AZSC) team in the Central Phoenix territory. The role delivers one-to-one, compassionate care primarily in the patient’s home or care facility, coordinating with physicians to manage daily living and disease management needs.

The candidate will develop therapeutic relationships, assess and plan care, and document meticulously while maintaining high professional standards.

Qualifications

• Graduate of accredited school of nursing.
• OASIS C experience.
• Minimum one year of recent medical, surgical or critical care experience.
• Excellent communication and interpersonal skills.
• Strong clinical assessment skills.
• Ability to manage time with attention to detail and follow-through.
