Job summary

UnitedHealth Group in Cottonwood, AZ is seeking a Registered Nurse Case Manager to deliver hospice care plans and coordinate patient care in home settings, supporting patients, families and the care team with compassionate, evidence-based practice.

You will conduct initial evaluations within 48 hours, develop and revise the Plan of Care, document progress and collaborate with physicians and staff to improve quality of life for patients facing life-limiting illness.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to reliable transportation.
• Preferred: 1+ years of hospice or clinical nursing experience.

Responsibilities

• Makes initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs; reassesses at least every 15 days.
• Initiates and coordinates the plan of care with the IDG and attending physician.
• Documents problems, goals, interventions, and patient/family response.
• Collaborates with patient/family, physician, and IDG for daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on patient condition.
• Function in diverse home settings regardless of background.
