Job summary

Legacy Hospice, Inc. is seeking a Registered Nurse Case Manager to deliver compassionate, in-home hospice care in Southaven, MS. You will assess patients, develop care plans, coordinate with the IDT, manage medications, and support families with empathy and expertise.

The ideal candidate is an RN with MS licensure, at least 1 year in hospice or home health, CPR/BLS, and a valid driver's license. This full-time, exempt role offers mileage reimbursement and growth opportunities within a

Qualifications

RN licensed in the state of employment (MS).
BSN or CHPN preferred.
Minimum 1 year of nursing experience in hospice or home health.
Valid driver’s license and reliable transportation.
Active CPR/BLS certification.

Responsibilities

Perform comprehensive patient assessments and create individualized care plans.
Manage a caseload of hospice patients, coordinating care with the interdisciplinary team.
Administer medications and treatments per physician orders.
Educate patients and families on symptom management and end-of-life care.
Monitor and update care plans based on ongoing patient assessments.
Provide emotional and clinical support to patients and caregivers.
Communicate with physicians, care partners, and community resources.
Maintain accurate, timely clinical documentation.
Participate in interdisciplinary team (IDT) meetings and on-call rotation.
Supervise ancillary staff and ensure continuity of care.
