Responsibilities

• Provide skilled nursing assessment and care in patients’ homes and communities.
• Develop and implement individualized plans of care.
• Assess patient and family needs and coordinate appropriate services.
• Collaborate with the interdisciplinary group (IDG).
• Communicate effectively with patients, families, physicians, and team members.
• Monitor medications, supplies, and equipment for appropriate utilization.
• Document care accurately and maintain compliance with hospice regulations and standards.
• Assist with On call requirements.
