Job summary

Daughters of Miriam Center for Nursing & Rehabilitation in Beachwood, OH is seeking an experienced Hospice RN Case Manager to deliver compassionate in-home nursing care. You will assess patients, develop care plans, and coordinate services in patients' homes and the community.

The role requires Ohio RN license, hospice/home health experience preferred, strong communication, and ability to work independently while collaborating with the IDG.

• Monitor medications, supplies, and equipment for appropriate utilization.
• Document care accurately and maintain compliance with hospice regulations and standards.
• Assist with On call requirements.
