Job summary

Outcome HC is seeking an experienced Hospice RN Case Manager to provide compassionate, high-quality end-of-life care in patients’ homes and communities around Beachwood, OH. You will develop individualized care plans and work with an interdisciplinary team to support patients and families.

The role emphasizes comfort, dignity, and quality of life, with on-call responsibilities and strong clinical judgment. A valid Ohio RN license and CPR are required.

Qualifications

• Current RN license in the State of Ohio.
• Hospice, home health, or community nursing experience preferred.
• CPR Certification (Required).
• Ability to commute to Beachwood, OH and surrounding communities.
• Willingness to travel to King David Campus and surrounding communities.

Responsibilities

• Provide skilled nursing assessment and care in patients’ homes and communities.
• Develop and implement individualized plans of care.
• Assess patient and family needs and coordinate appropriate services.
• Collaborate with the interdisciplinary group (IDG).
• Communicate effectively with patients, families, physicians, and team members.
• Monitor medications, supplies, and equipment for appropriate utilization.
• Document care accurately and maintain compliance with hospice regulations and standards.
• Assist with On call requirements.
