Qualifications

• RN licensed in the state where care is provided.
• Experience in hospice, home health, geriatrics or medical/surgical nursing.
• Commitment to the hospice philosophy of care.

Responsibilities

• Receives patient assignments from the Patient Care Manager and coordinates care.
• Communicates with physicians, hospice staff and other agencies as needed.
• Maintains up-to-date patient records and plans of care.
• Instructs patient and caregiver on medications, hygiene, lifting and safety.
• Provides 24/7 coverage to patients and families in rotation.
• Attends team meetings and care plan conferences.
