Job summary

Good Shepherd Hospice is seeking a compassionate Registered Nurse Case Manager to serve patients and families in Durant and surrounding communities. The RN will perform nursing assessments, planning and care to maximize comfort and health, respond to calls, and conduct patient visits as needed.

Responsibilities include coordinating with physicians and staff, documenting care plans, instructing patients and caregivers, and providing 24/7 coverage as needed.

Qualifications

• RN licensed in the state where care is provided.
• Experience in hospice, home health, or geriatrics.
• Commitment to hospice philosophy of care.

Responsibilities

• Receives patient assignments and coordinates care with physicians and hospice staff.
• Maintains communication with patients' families and documents plans of care.
• Instructs patients and caregivers on medications, daily care, and safety.
• Provides after-hours admissions and coverage as needed.
