Job summary

Good Shepherd Hospice is seeking a compassionate Registered Nurse Case Manager to serve patients and families in Grove and surrounding communities. The RN will perform nursing assessments, plan care, and coordinate with physicians and staff to maximize comfort and health.

Regular family communication and timely after-hours admissions are essential. The role requires an active RN license in Oklahoma and prior hospice, home health, geriatrics, or acute care experience.

Qualifications

• RN licensure in the state where care is provided.
• Experience in hospice, home health, geriatrics, or medical/surgical nursing preferred.
• Demonstrated commitment to hospice philosophy of care.

Responsibilities

• Receives patient assignments from the Patient Care Manager or Case Manager and coordinates care with attending physicians and hospice staff to optimize patient outcomes.
• Maintains regular communication with the attending physician regarding patient/family care.
• Documents data on physical, psychological, social and spiritual factors and updates the Plan of Care accordingly.
• Instructs patient and caregiver on medication administration, adverse reactions, personal care, lifting and mobility.
• Participates in hospice team meetings and care plan conferences; provides support at time of death.
• Ensures coverage for 24/7 on-call needs and participates in after-hours admissions as required.
• Evaluates hospice aides and LPNs regularly to maintain care quality.
• Develops ongoing understanding of hospice philosophy through study and training.
