ExcelaCare Las Cruces Hospice is seeking an experienced RN Case Manager to provide hospice care at patients' homes or in facilities. The role involves coordinating with a multidisciplinary team, managing patient plans, and delivering compassionate end-of-life care.

The RN will administer medications, assess patients, educate families, and participate in on-call rotations, ensuring quality care and compliance with agency policies.

Qualifications

• Graduate of an accredited school of nursing.
• Current valid State RN license.
• Current CPR certification required.
• Driver's license and proof of auto liability insurance; no OIG Excluded Provider listing.

Responsibilities

• Coordinate hospice care for clients in home or facility settings with the multi-disciplinary team.
• Administer medications and treatments per policy and guidelines.
• Develop and implement Plan of Care with physician orders and patient needs.
• Document patient status and interventions accurately and timely.
• Educate patients and families on healthcare plans, symptom control, and end-of-life options.
• Attend on-call rotations and make visits as scheduled.
