Job summary

Cadre Family Companies is seeking an RN Case Manager to be the primary clinical advocate for patients, coordinating personalized hospice care with the interdisciplinary team.

You will assess needs, develop care plans, manage symptoms, and educate patients and caregivers while navigating goals of care with compassion and respect.

This role emphasizes collaboration with physicians, social workers, chaplains, aides, and volunteers to deliver dignified end-of-life care.

Qualifications

• Current, unrestricted RN license in the state of practice.
• Minimum of one year of nursing experience.
• Valid driver's license, reliable transportation, and ability to travel throughout the service area.

Responsibilities

• Perform comprehensive nursing assessments of patients’ physical, emotional, and psychosocial needs.
• Develop and manage individualized plans of care with the interdisciplinary team.
• Monitor patient progress and adjust care plans to maximize comfort and quality of life.
• Serve as the primary clinical resource for patients and families throughout the hospice journey.
• Educate patients and caregivers on medications, symptom management, and disease progression.
• Collaborate with physicians, social workers, chaplains, aides, and volunteers to coordinate transitions of care.
