Responsibilities

• Assess patients' physical, emotional, and psychosocial needs.
• Develop, implement, and review individualized care plans.
• Manage pain and other distressing symptoms.
• Educate patients and families about care options and advance directives.
• Collaborate with physicians, nurses, social workers, and chaplains.
• Maintain accurate clinical documentation and participate in QAPI.
