Vitality Hospice & Palliative Care is seeking a Registered Nurse Case Manager to join a compassionate team in Bucks County, PA. The RNCM will lead admissions, assessments, and plan-of-care development within an interdisciplinary team in a hospice setting.

Ideal candidates hold an active PA RN license, hospice or home health experience, strong communication skills, and a willingness to travel within the service area. Comprehensive benefits are offered with a supportive work culture.

Qualifications

• Current RN License in Pennsylvania (required).
• Clinical nursing experience in hospice or home health preferred.
• Strong communication, organizational, and interpersonal skills.
• Valid driver’s license, reliable vehicle, and current auto insurance.
• Ability to travel within the assigned service area.

Responsabilités

• Conduct patient admissions and ongoing nursing assessments.
• Develop and implement individualized plans of care based on patient needs and goals.
• Collaborate closely with hospice physicians and the interdisciplinary team (IDT).
• Supervise and coordinate care with home health aides, social workers, and chaplains.
• Educate patients and caregivers about disease progression, symptom management, and available resources.
• Ensure timely and accurate documentation in compliance with hospice standards.
• Promote a culture of excellence, empathy, and accountability in every interaction.
