Job summary

Vitality Hospice & Palliative Care in New Jersey is seeking a Registered Nurse Case Manager (RNCM) to coordinate patient care across the IDT, admit new patients, and conduct ongoing nursing assessments in home settings. This full-time role requires NJ RN license, strong communication, and the ability to travel within the service area.

You will develop individualized plans of care and document in compliance with hospice standards, with competitive salary and comprehensive benefits.

Qualifications

• NJ RN license required
• Hospice or home health experience preferred
• Strong communication, organizational, and interpersonal skills
• Valid driver’s license and reliable vehicle
• Ability to travel within service area

Responsibilities

• Conduct patient admissions and ongoing nursing assessments
• Develop and implement individualized plans of care based on each patient's needs and goals
• Collaborate closely with hospice physicians and the interdisciplinary team (IDT)
• Supervise and coordinate care with home health aides, social workers, and chaplains
• Educate patients and caregivers about disease progression, symptom management, and available resources
• Ensure timely and accurate documentation in compliance with hospice standards
• Promote a culture of excellence, empathy, and accountability in every interaction
