Chandler Hall is seeking an experienced Registered Nurse Case Manager to join our Home Hospice team in the Newtown area. You will coordinate nursing care for an assigned caseload, collaborating with patients, families, physicians, and the interdisciplinary team to manage symptoms and support goals of care.

Qualificações

• Current RN license in Pennsylvania.
• At least 1 year of hospice nursing experience.
• Certified Hospice and Palliative Nurse certification preferred.
• Strong knowledge of hospice philosophy, end-of-life care, and pain/symptom management.
• Ability to complete patient-centered assessments and develop plans of care.
• Independent in community settings while collaborating with the interdisciplinary team.

Responsabilidades

• Manage and coordinate care for an assigned hospice caseload.
• Complete comprehensive and ongoing nursing assessments.
• Develop, implement, evaluate, and update individualized plans of care with patient, family, physician, and team.
• Provide scheduled and as-needed nursing visits based on patient needs.
• Assess and manage pain, symptoms, medications, and safety concerns.
• Educate patients and caregivers about medications and symptom management.
• Coordinate care with physicians, facilities, pharmacies, and equipment providers.
• Participate in interdisciplinary team meetings and care planning.
• Support hospice eligibility reviews and complete admissions, certifications, and discharges.
