Job summary

Chandler Hall’s Home Hospice team seeks an experienced Registered Nurse Case Manager to coordinate and deliver skilled end-of-life care in patients' homes throughout Bucks County, Montgomery County, and Northeast Philadelphia.

The RNCM will assess patient needs, develop care plans with families and the interdisciplinary team, provide visits, manage symptoms, and ensure compassionate, dignified care in the comfort of the patient's own home.

Qualifications

• Current Pennsylvania RN license.
• Minimum 1 year hospice nursing experience.
• Certification in Hospice and Palliative Nursing preferred.
• Strong knowledge of end-of-life care and pain/symptom management.
• Ability to assess patients and develop care plans.
• Excellent communication with patients, families, and teams.

Responsibilities

• Coordinate and manage an assigned hospice caseload.
• Conduct comprehensive nursing assessments.
• Develop and update individualized care plans with patient, family, physician, and team.
• Provide scheduled visits and symptom management.
• Educate patients and caregivers on medications and safety.
• Document accurately and timely in medical records.
• Collaborate with physicians, facilities, pharmacies, and other care-team members.
