Job summary

Vitality Hospice & Palliative Care is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our clinician-owned team serving patients and families in Bucks County, PA. This full-time role offers a Monday–Friday schedule with occasional evenings/weekends, a salary range of $85,000–$100,000, and a supportive work environment focused on high‑quality end-of-life care.

Qualifications

• Current Registered Nurse (RN) license in Pennsylvania.
• Hospice or home health experience preferred.
• Strong communication and organizational skills.
• Valid driver’s license and ability to travel within service area.

Responsibilities

• Admissions and ongoing nursing assessments.
• Develop and implement individualized care plans.
• Collaborate with hospice physicians and IDT.
• Supervise and coordinate care with home health staff.
• Educate patients and caregivers on disease progression and symptom management.
• Ensure timely documentation in accordance with hospice standards.
