Job summary

Beth Israel Lahey Health at Home in Woburn, MA is seeking an experienced Registered Nurse to plan, organize and direct home care services. The RN draws on community resources to meet patients and families’ needs in homes and communities, following home care regulations and payer guidelines.

Responsibilities include completing initial assessments, initiating and revising the plan of care, administering medications, supervising home health aides every two weeks, and ensuring OASIS and EPIC

Qualifications

• Active Massachusetts RN license.
• Active BLS certification.
• 1 year of direct care nursing experience.
• OASIS and EPIC experience preferred.

Responsibilities

• Complete and document initial patient assessment to determine home care needs.
• Initiate and revise the plan of care as patient status changes.
• Administer medications and treatments as prescribed.
• Instruct, supervise, and evaluate home health aide care every two weeks.
• Identify discharge planning needs as part of the care plan.
• Maintain HIPAA confidentiality and ensure OASIS/EPIC documentation is timely.
