Job summary

Optimal Hospice is seeking a Registered Nurse Case Manager to deliver comprehensive hospice care. You will assess patients and families, plan and coordinate care, administer medications, and educate families as part of an interdisciplinary team.

The role emphasizes compassionate, patient-centered service with ongoing documentation and collaboration to ensure comfort and quality of life for patients and their families.

Qualifications

• Registered nurse with current licensure to practice in the state.
• Graduate of National League for Nursing accredited school of nursing.
• Maintains a current CPR certification.
• Current valid driver's license.

Responsibilities

• Completes initial, comprehensive and ongoing patient/family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and implements care plans based on assessments and diagnoses.
• Regularly re-evaluates patient and family needs and revises plans as necessary.
• Initiates preventive and rehabilitative nursing procedures and administers prescribed medications.
• Counsels patients and families on nursing needs and health instructions.
• Maintains accurate clinical notes and communicates changes with physicians.
• Works with the interdisciplinary team to meet emotional and care needs of patients and families.
