Job summary

Patriot Healthcare in Conway, South Carolina, is seeking an experienced Registered Nurse Case Manager to join our hospice team. You will coordinate and direct hospice services according to physician orders, work with the interdisciplinary team, and provide compassionate end-of-life care that honors patient dignity.

The ideal candidate holds a Nursing degree and an SC RN license with at least 1 year of nursing experience in healthcare settings; hospice background is preferred.

Qualifications

Nursing degree from an accredited program.
Licensed RN in South Carolina.
1 year of hospital/long-term care experience preferred.
Hospice experience preferred.

Responsibilities

Coordinate hospice services per physician orders and patient plan of treatment.
Serve on Interdisciplinary Team (IDT) and attend meetings.
Provide nursing care documentation to support billing and quality assurance.
Supervise Hospice Aide care and CNA schedules.
Maintain patient records and communicate with facility staff and families.
Report to the Director of Clinical Services and coordinate with physicians.
