Crescent Hospice is seeking a compassionate Hospice Registered Nurse Case Manager (RNCM) for full-time coverage in the Augusta area. The role focuses on direct patient care, care coordination, and guiding families through the hospice journey with a patient-centered approach.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year RN experience in a medical-surgical/acute care setting; hospice experience is a plus.
• Proficient with computers and EMR systems.
• CPR certification if required by state.
• Valid driver’s license, vehicle insurance, and reliable transportation.

Responsibilities

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries with compassion.
• Collaborative Care Champion: Partner with Clinical Director to assess eligibility and suitability for hospice services.
• Nurturing Direct Care: As an RN Case Manager, coordinate care while ensuring patient comfort and quality of life.
• Nursing with Heart: Deliver top-notch care rooted in proven nursing principles.
• Educator and Advocate: Educate patients and families to support active participation in personalized care plans.
• Call of Compassion: Share call duties to be there when it matters most.
