Job summary

Crescent Hospice in Augusta, GA seeks a Per Diem Hospice RN Case Manager to deliver hands-on nursing care, coordinate services, and guide patients and families through the hospice journey.

As part of our interdisciplinary team, you will assess needs, educate families, and advocate for comfort and quality of life, with a focus on compassion, clinical excellence, and reliable call coverage.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient in computer use and EMR systems.
• Driver's license and reliable transportation.

Responsibilities

• Respond to clinical referrals with compassionate, timely care.
• Assess client eligibility for hospice services with Clinical Director.
• Deliver direct nursing care to support comfort and quality of life.
• Provide skilled nursing rooted in evidence-based practice.
• Educate patients and families to align care plans with goals.
• Share call duties to ensure access to care when it matters most.
