Job summary
Crescent Hospice is seeking a dedicated Registered Nurse Case Manager to join our clinical team in Anderson, SC. This full-time role delivers direct hospice nursing care, coordinates services, and supports patients and families through their hospice journey with compassion and clinical excellence.

Qualifications
- Current unencumbered RN license.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
- Proficient computer skills; experience with EMR.
- CPR certification if required by state.
- Valid driver's license and reliable transportation.
Responsibilities
- Deliver hands-on hospice nursing care and coordinate services.
- Guide patients and families through the hospice journey.
- Collaborate with the interdisciplinary care team to ensure quality care.
- Respond to clinical referrals with compassionate expertise.
- Provide education and support to patients and families.
- Participate in on-call duties as required.
