Job summary

Bristol Hospice is seeking a Registered Nurse Case Manager to provide compassionate, whole-person hospice care in the Richmond, VA area. You’ll develop individualized care plans, monitor patients, and educate families while coordinating with an interdisciplinary team.

Eligible candidates hold an active RN license, CPR certification, a valid driver's license with reliable transportation, and strong communication skills.

Qualifications

• Must hold a valid RN license in the state of work.
• Current CPR certification.
• Excellent observational and written/verbal communication skills.
• Valid driver’s license and reliable transportation.
• Ability to educate patients and families on care techniques.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver one-on-one support to patients and families, ensuring comfort and dignity.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs and clinical notes.
• Administer physician-ordered medications and treatments with empathy.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Maintain up-to-date documentation and communicate changes promptly.
