Job summary

Empyrean Hospice hires Registered Nurse Case Managers to plan, organize, and direct hospice care with emphasis on community health education for patients and families. The role focuses on comprehensive assessments, care planning, and coordination of high-quality end-of-life care across a dedicated care team.

Responsibilities include developing goals with patient and family, supervising aides, documenting care, and communicating with physicians to ensure continuity of care and ethical practice.

Qualifications

• RN licensure and CPR certification are required.
• Bachelor's degree preferred; hospice care experience adds value.
• 1–2 years acute care in institutional setting and 1–2 years in home care.
• Ability to supervise hospice aides when needed.

Responsibilities

• Assess patient and family needs; develop and implement care plans.
• Coordinate care for assigned caseload; liaise with physicians and team.
• Counsel patients and families; provide care instructions and support.
• Maintain clinical notes and reports; ensure continuity of care.
