Job summary

Empyrean Hospice is seeking a Registered Nurse Case Manager to plan, organize, and direct hospice care with emphasis on community health education. You will assess patients and families, develop individualized care plans, and coordinate care with the interdisciplinary team.

Responsibilities include counseling patients and families, assisting with daily living activities, maintaining accurate clinical notes, and ensuring continuity of care with physicians.

Qualifications

• Licensed RN with current state licensure and CPR certification.
• Bachelor’s degree in nursing; 1–2 years acute care plus 1–2 years home care experience preferred.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments.
• Develop and implement individualized care plans with patient/family involvement.
• Coordinate care across the hospice team and maintain documentation.
• Counsel patients and families on nursing needs and care instructions.
• Assist with activities of daily living and support patient self-sufficiency.
• Communicate changes in patient condition to physicians and team.
