Job summary

Heart of Hospice, part of LHC Group and UnitedHealth Group family, is seeking a Per Diem Registered Nurse Case Manager to deliver hospice care and manage Plan of Care for patients. The role requires evaluating performance and aligning care with policy objectives to ensure quality outcomes.

The RN will perform initial evaluations, coordinate care, document progress, and collaborate with families and the IDG. Travel and scheduling support are included where needed.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with clinical notes.
• Documents problems, goals, interventions, and patient/family response.
• Collaborates with patient/family, attending physician and IDG daily.
• Adheres to hospice policies to assure quality patient care.
