Job summary

Kindful Health LLC seeks a compassionate Registered Nurse to provide hospice care in patients' homes across the Austin area. The role emphasizes comprehensive assessments, care planning, and education for patients and families.

Requires an active RN license, a valid driver's license, and at least two years of clinical RN experience; hospice experience is preferred. The nurse must be able to travel locally, lift up to 50 pounds, and work a flexible schedule.

Qualifications

• Graduate from an accredited nursing program (diploma/ADN/BSN).
• Current RN license and valid driver’s license.
• Two years RN experience in a clinical setting; hospice experience preferred.
• Excellent communication and interpersonal skills; English literacy required.
• Flexible schedule with local travel; ability to lift up to 50 lbs.

Responsibilities

• Perform initial and ongoing comprehensive nursing assessments in patient homes.
• Develop and document plans of care with the Interdisciplinary Group.
• Educate patients and families on disease process, self-care, nutrition, and end-of-life care.
• Coordinate care with IDG/Team and referring agencies; provide training as needed.
• Respond to on-call duties and communicate changes to the care team.
