Job summary

Halcyon Home is seeking a dedicated Full-Time Hospice RN Case Manager to join our team in Austin, TX. This role focuses on compassionate patient care, comprehensive assessments, and care coordination to support families during end-of-life care.

The ideal candidate holds an RN license in good standing, has strong clinical and communication skills, and can work independently within our team. Homecare Homebase EMR experience is a plus; hospice experience is preferred.

Qualifications

• Current RN license in good standing with the state board.
• Experience or knowledge in hospice or related healthcare settings preferred.
• Excellent clinical assessment and communication abilities.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs for hospice care.
• Develop and update the plan of care to ensure quality and continuity of care.
• Educate patients and families about disease, end-of-life care, and self-care strategies.
• Provide clinical direction to aides and LPNs to ensure consistent service.
• Monitor cases for third-party payor compliance and coordinate care with the IDG.
