Job summary

CareFor in Austin, TX is seeking a compassionate Registered Nurse to join our hospice care team as a Hospice RN Care Manager. You will build trusting relationships in patients' homes, deliver comprehensive, compassionate care, and help families navigate the final days with comfort and dignity.

You will assess, plan, and implement individualized care plans for multiple patients across North Austin communities, coordinate with the interdisciplinary team, document progress, and educate families on

Qualifications

• Current RN license active in Texas or NLC rules applicable.
• Minimum 2 years nursing experience; hospice/palliative care valued.
• Reliable transportation to North Austin area (Pflugerville, Round Rock, Cedar Park, Leander).
• Strong communication and documentation skills.
• Dedication to compassionate, patient- and family-centered care.

Responsibilities

• Conduct initial and ongoing comprehensive assessments to develop care plans reflecting patient needs.
• Plan and document personalized care plans, revising as needed.
• Educate patients and families, guide end-of-life care, and train staff as needed.
• Coordinate with interdisciplinary team to prioritize and deliver care.
• Maintain clinical documentation meeting payer and regulatory standards.
• Communicate effectively with patients, families, and care team.
• Manage weekly visit schedule and be available for urgent needs.
• Participate in hospice in-service programs and QAPI initiatives.
