Job summary

Hospice Austin in Spicewood/Austin, TX is seeking a Registered Nurse to provide comprehensive hospice care in homecare and facility settings. You will perform admissions visits, conduct assessments, develop plans of care, manage symptoms, and educate patients, families, and facility staff on hospice care.

The RN will supervise CNAs/LVNs/homemakers, coordinate with physicians and the interdisciplinary team, document accurately, and support discharge planning.

Qualifications

• Graduation from an approved nursing program.
• Unrestricted RN license by the Texas Board of Nurse Examiners.
• Valid Texas driver’s license and CPR certification.

Responsibilities

• Provide professional nursing care for hospice patients in homecare and facility settings.
• Perform admissions visits and comprehensive assessments, develop and update plans of care.
• Manage symptoms and provide end-of-life care education to patients and families.
• Coordinate with physicians, IDT, and facility staff to ensure smooth transitions of care.
• Supervise CNAs, LVNs, and homemakers; document in agency records timely.
• Participate in discharge planning and ensure accurate charting.
