Job summary

Big Bend Hospice in Florida is seeking an experienced RN Case Manager II to lead admission and coordination across care settings. Qualifications

• Florida RN license required.
• Minimum 2 years inpatient nursing experience preferred.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients for admission using approved criteria.
• Conduct pre-assessment and admission visits in home/hospice settings.
• Coordinate with physicians and intake teams to promote timely admissions.
• Provide education on hospice philosophy, eligibility, and services.
• Develop IDG plan of care to meet patient and family needs.
• Ensure timely, accurate documentation in EMR; align with regulatory requirements.
• Mentor staff and participate in IDG meetings and quality initiatives.
